Support Department: Case Management
Schedule: Full-Time 08:00am-16:30pm
Reports To: Director of Case Management
Summary: Plan, design, implement, coordinate and evaluate case across the continuum to facilitate and insure quality, clinical and cost outcomes.
Requirements: Current CA RN license. Prev. Exp as a Case Manager in an acute care setting. Must have working knowledge of Interqual IS/SI. Experience working with computers. 8 hour shift - Monday - Friday and rotating weekends.
