RN, Case Manager II

Schedule: Full-Time 08:30 AM -17:00 PM

Reports To: Director of Case Management

Summary: Plan, design, implement, coordinate and evaluate care across the continuum to facilitate and ensure quality, clinical, and cost outcomes.

Requirements: Current CA RN license. Prev. Exp as a Case Manager in an acute care setting. Must have a working knowledge of Interqual IS/SI. Experience working with computers.
