Position:  RN, Case Manager II

Schedule: Full-Time 8 AM - 4:30 PM

Reports To: Director of Case Management

Summary: Plan, design, implement, coordinate and evaluate case across the continuum to facilitate and insure quality, clinical and cost outcomes.

Requirements: Current CA RN license and current BLS. Two years previous experience as a Case Manager in an acute care setting preferred. Must have working knowledge of Interqual IS/SI. Experience working with computers.
