RN, Case Manager II

Support Department: Case Management

Management: No

Schedule: Per Diem 08:00AM -16:30PM

Reports To: Director of Case Management

Summary: Plan, design, implement, coordinate and evaluate case across the continuum to facilitate and insure quality, clinical and cost outcomes.

Requirements: Current CA RN license. Prev. Exp as a Case Manager in an acute care setting. Must have working knowledge of Interqual IS/SI. Experience working with computers.
