RN, ER Case Manager
Support Department: Case Management
Schedule: Full-Time 06:00 AM - 18:30 PM
Reports To: Director of Case Management
Summary: Assure appropriate initial medical necessity admissions. Provide a level of care for patient transition from the emergency room. Coordinate care to facilitate and ensure good clinical and quality patient outcomes.
Requirements: Must have current CA RN Licensure with a preference of 2 years of Case Management experience in an acute care facility with one of those years working in the emergency room to navigate and connect successfully with outside provider networks (Health Plans and any IPA's). BSN preferred (individuals without a BSN will be required to complete the degree within 5 years of their hire date). Must have a working knowledge of Interqual IS/SI, alternative level of care, and discharge criteria. Current AHA BLS required. Case Management Certification preferred. Must have experience working with computers.
